手术比药物更早知道的事

你总体上如何评价 GLP-1 这类药物?它们是减肥的完美解决方案吗?
肥胖是一个严重的问题,在 GLP-1 问世之前,伴随肥胖而来的各种代谢问题基本上没有得到有效处理,所以从这个角度看,这类药物非常了不起。它并不完美,存在局限,但拥有它们总比没有好。依从性是个问题:每个月有超过 100 万人停用 GLP-1,大约 30% 的人在第一个月内停药,到年底约有 50%。这种模式并非 GLP-1 独有——降压药和降胆固醇药也有同样的脱落现象。
患者停用 GLP-1 的主要驱动因素是什么——是副作用,还是不愿给自己注射?
注射疲劳可能是原因之一——有些人不想每周给自己打一针,每天口服的药也容易忘记吃。剂量爬坡到有效剂量同样有问题:高剂量下恶心,还有肌肉流失。长期来看,你摄入的是超生理剂量的物质,其长期影响尚不清楚;尽管收益大于代价,这种不确定性仍让一些患者不安,也可能是他们停药的原因。在我的内镜诊疗实践中,超过 85% 的患者已经用过 GLP-1,要么用得吃力,要么已经停药。
是否有好的研究表明,力量训练可以抵消标准剂量或微剂量 GLP-1 药物造成的肌肉流失?
我不了解有哪项研究以此为主要终点,但力量训练在维持肌肉方面确实起主要作用——这一点对任何减重方式都成立:第一代药物、任何外科或内镜减重手术,不只是 GLP-1。坚持力量训练时,身体意识到它需要这些肌肉,不会在人减重时把它们消耗掉。热量缺口下,身体会想办法维持能量水平,你不希望它为此分解肌肉。
